A Subsidy Health and Safety inspection was conducted on 10/20/17 to review supplemental health and safety requirements. There were 30 children, infants to preschool age, in care at the time of the inspection. There were 5 staff members working directly with the children in their classrooms. All groups were within the proper ratio. The children throughout the facility were engaged in activity. The infants were crawling on the floor and sitting in infant seats. The toddlers were playing on the floor with their teacher. The preschoolers were having circle time reviewing the colors in Spanish. The two year olds were playing with animal figurines and playhouses. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Medication is administered.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a review of a sample of children's records, the facility did not ensure that a written allergy plan was obtained for a child with a diagnosed food allergy. EVIDENCE: The record of child#1 did not contain a written allergy plan which includes instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The allergy care plan has been given to the child's parent to give to the physician to complete. She must submit by Friday 10/27/17.
