Comments:
An unannounced monitoring inspection was conducted today, October 12, 2017 to determine the center's compliance with licensing standards. The inspector arrived at 10:00 am and concluded the inspection at 3:45 pm. Upon the inspectors arrival, children were observed in a variety of activities. Children in the upper and lower elementary classrooms were working in their work cycles which included making juice from beets and other fresh vegetables, calculating square roots with the peg board, and sorting blocks by layers. Outside on the playground, the primary classes played with balls, frisbees and on the climbing equipment. Later the children had lunch and prepared for nap. After nap the primary classrooms played with blocks, puzzles, sensory table/practical life and colored skeletons. A small group also were observed sewing a felt pumpkin together. A class of children two years in age has been added on the first floor. Children were observed painting invitations for a upcoming grandparents day event. Children provide their own meals which were labeled with names. All areas of the facility were inspected. The facility has age and stage appropriate materials and supplies. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule, allergy information and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The playground was not inspected due to rain. The inspector interacted with staff and children in care. Ten staff records and ten children's records were reviewed during today's inspection (see violation notice). The following ratios were observed in each classroom: Lower elementary: (6-9 years): 25 children with 2 staff. Upper elementary: (9-12 years) 16 children with 1 staff. Primary I: (2.5-5 years) 20 children with 3 staff. Primary II: (2.5-5 years) 20 children with 3 staff. Pre-Primary: (2-3 years) 9 children with 2 staff. Middle School: 10 children with 2 staff. The last evacuation drill was conducted on 09/18/2017. A shelter-in-place drill for 2017 year was conducted on 05/12/2017. A second drill will need to be conducted by the end of the school year. The facility has current liability Insurance. The Emergency Preparedness plan is current. Health Inspection: 12/19/2016 Fire Inspection: 10/09/2017 Medications were reviewed. Multiple staff have current MAT certification. Staff have current CPR & First aide training along with Daily Health Observation training. The violations from the previous inspection on March 21, 2017 were checked for corrections. There was one repeat violation during today's inspection. Standard #: 22VAC40-185-160-A
Description: Based on review of staff records, the facility did not have documentation of a negative tuberculosis screening for staff no later than 21 days after employment. Evidence: Staff #7, hired 06/01/2017, did not have documentation of a tuberculosis screening on record.

Plan of Correction: Per the Administrator: I will have the staff obtain documentation of the tuberculosis screening or obtain a new one if needed.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, The facility did not obtain two or more references before employment. Evidence: Staff #8, hired 08/28/2017, had documentation of two references dated 10/04/2017 and 10/05/2017 exceeding the requirement to obtain before employment.

Plan of Correction: Per the Administrator: We will ensure all references are obtained before employment.

Standard #: 22VAC40-191-60-C-1
Description: Based on review of staff records, the facility did not obtain an original criminal history record within 30 days of employment. Evidence: Staff #5, hired 02/05/2017, did not have a completed criminal history record on file.

Plan of Correction: Per the administrator: The criminal history was requested however it was returned for missing information. We overlooked re-submitting it. A new request will be made today.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, The facility did not obtain a central registry finding within 30 days of employment. Evidence: Staff #10, hired 10/06/2015, had a central registry finding dated 12/29/2015 exceeding the 30 days allowance.

Plan of Correction: Per the Administrator: We will be sure to mail all background request off at the time of hire to ensure they are received in the time period allowed.

Standard #: 63.2-1720.1-B-3
Description: Based on review of staff records, The facility did not obtain a child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: 1. Staff #1, hired 07/10/2017, did not have documentation of results of a child abuse and neglect registry by other states in which that staff resided in the preceding five years at the time of record review. 2. Staff #2, hired 11/16/2016, did not have documentation of results of a child abuse and neglect registry by other states in which that staff resided in the preceding five years at the time of record review. 3. Staff #3, hired 08/21/2017, did not have documentation of results of a child abuse and neglect registry by other states in which that staff resided in the preceding five years at the time of record review. 4. Staff #6, hired 08/17/2017, did not have documentation of results of a child abuse and neglect registry by other states in which that staff resided in the preceding five years at the time of record review. 5. Staff #7, hired 06/01/2017, did not have documentation of results of a child abuse and neglect registry by other states in which that staff resided in the preceding five years at the time of record review. 6. Staff #9, hired 06/02/2015, did not have documentation of results of a child abuse and neglect registry by other states in which that staff resided in the preceding five years at the time of record review.

Plan of Correction: Per the Administrator: I have printed out all state registry forms for each staff. I will send the requests off today.
