Comments:
SHSI inspection conducted with the director and two staff. 12 Children were in care with three staff. The age range of the children was five years to ten. Children were observed during free play. Inspection time 7:00am-8:00am

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on review of not all children with a food allergy, had a written allergy plan. Evidence: Child A with a food allergy and medications stored at the center did not have a food allergy plan.

Plan of Correction: We have been contact with parent and given them the paper work.
