22VAC40-191 Background Checks (22VAC40-191)
An unannounced subsidy health and safety inspection was conducted with the director beginning at 2:00 PM and ending at 4:25 PM. There were fifty-five children in care, ages four months through four years old, in care during the inspection. The infants were observed singing songs and playing with manipulatives. One infant was observed being bottle fed. The toddlers were observed eating snack which consisted of apples, cheese crackers, and water. The two year old class was observed having a book read to them. The three year old class was observed playing with Legos and making slime. The Montessori class was observed using chalk and doing a spider art project. Five children?s records and five staff records were reviewed and in compliance. Two medications were reviewed and in compliance. The emergency drill logs were reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed an in compliance. The most recent health inspection was documented as March 1, 2017. The most recent fire inspection was documented as January 10, 2017. Standard #: 22VAC40-185-270-A Description: Based on observation, not all of the areas and equipment of the center, inside and outside, were not maintained in a safe and operable condition. Evidence: There were two bicycles with missing pedals on the large playground. There pieces of splintered wood on the front of the train located on the small playground which created a scratching hazard. One of the toilets in the bathroom of the three year old classroom was broken; the tank of the toilet was leaning against the wall and not able to flush. Plan of Correction: The bikes on the playground were removed immediately. A work order was put in for the splintered wood on the train. It was fixed 10/9. There were two other working toilets in the classroom. A work order was put in prior to this visit. It was fixed 10/9.
Standard #: 22VAC40-185-280-B Description: Based on observation not all hazardous substance were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a bottle of cleaning solution left on a shelf in the school age classroom. The laundry, room was left unlocked; there were several bottles of cleaning solution in the room. The labels on the cleaning solution stated keep out of reach of children. Plan of Correction: The cleaning solution was stored out of the reach of children on a shelf above the sink. The director removed it immediately and locked it in a locked cabinet. The laundry room door was accidently left unlocked by a staff member. The director locked it immediately. The cleaning products that are stored in the laundry room were stored on a shelf out of reach of children
Standard #: 22VAC40-185-500-A Description: Based on observation, not all of the hand washing procedures were followed as required. Evidence: The children in the toddler classroom did not wash their hands with soap and running water or disposable wipes before and after eating snack. Plan of Correction: There were 5 children present the day of the visit. The director had to remind the teacher to have the children wash their hands after snack. Staff were retrained on proper hand washing procedures.
Standard #: 22VAC40-185-570-E Description: Based on observation, not all refrigerated infant formula was labeled with the date as required. Evidence: On October 5, 2017, one bottle in infant A's classroom was dated September 29, 2017 and two bottles in infant B's classroom were dated September 5, 2017. Plan of Correction: Parent had dated the bottles on a piece of notebook paper when he came in that morning and accidently wrote 9/5 instead of 10/5. The director will retrain teachers to make sure they are checking all bottles are labeled with child's name and correct date upon arrival.
