A Subsidy Health and Safety inspection was conducted on 10/24/2017 to review supplemental health and safety requirements. here were 41 children in care with ten staff supervising the children. The inspector arrived at 12:00 P.M. and departed the program at 2:30 PM. Five children's and twelve staff's record were reviewed during the inspection. Children were observed: nap, beginning of snack and free play. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.
Standard #: VENDSUB-000-012-E Description: Based on review 15 staff's records, the center failed to ensure that all staff within 30 days of hire had first aid and CPR. Evidence: 1. Staff # 1, 4, 5, 8, 9, 10, 12, 13 and 14 did not have first aid and CPR within 30 days hire. Staff # 1 DOH 9/18/2017, Staff # 4 DOH 9/7/2017, staff #5 DOH 9/12/2017, staff # 8 DOH 8/28/2017, staff # 9 DOH 9/11/2017, staff # 12 DOH 8/18/2017, staff # 13 DOH 9/7/2017 and staff # 14 DOH 9/17/2017. Plan of Correction: There was a class scheduled and the instructor cancelled the class. The program director is in the process of rescheduling and or finding a new class.
