Comments:
An unannounced monitoring visit and facility tour was conducted from 10:20 am-11:45 am. At the time of inspectors' arrival there were 75 toddler-6 year old children in care with 8 staff members and 2 administrators. Children were observed participating in Montessori activities/instructional time in the classroom, transitioning between activities and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are noted on the violation notice and were discussed with the Director during exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-140-A
Description: Based on record review it was determined that the center did not ensure that each child had a physical examination by or under the direction of a physician within the required time frame. Evidence: The record for child #3(enrolled 9/6/16) did not contain documentation of a physical examination by or under the direction of a physician.

Plan of Correction: Physical examination documentation was requested from the parent during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There were areas of paint/laminate that had bubbled up and peeled off of the window leaving a rough surface in the toddler classroom. 2. There were vinyl gloves and hand wipes accessible to children in the bathroom of Children's House III.

Plan of Correction: 1. Window will be repaired by 11/3/17. 2. Items were removed and made inaccessible to children during this inspection.

Standard #: 22VAC40-185-330-B
Description: Based on measurement it was determined the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: In the outdoor play areas there was: -Less than 1" of resilient surfacing in the fall zones of the round nest style swing and monkey bars -Less than 3" of resilient surfacing in the fall zones of the climbing wall and geo dome

Plan of Correction: Resilient surfacing will be added in all fall zones.
