- Technical Assistance:
A Subsidy Health and Safety inspection was conducted on 10/25/17 to review supplemental health and safety requirements. The inspection started at 11:30 and concluded at 2:15. A licensing inspection was also conducted this date, the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Children Ratios Observed: 2-3 years: nine with two staff 3-4 years: 11 with two staff 4-5 years: 13 with two staff Five staff records reviewed. Five children records reviewed. Medication checked for two children. Classroom observations made in all classes. Conversations were held with all staff and several children.
Standard #: VENDSUB-000-006-B-11 Description: Based on review of children records the center did not have all requirements on file. Evidence: On 10/25/17, child #1 had an allergy listed on the enrollment form but did not have a written allergy care plan. Plan of Correction: Per Program Director, will contact parent regarding this information.
Standard #: VENDSUB-000-012-E Description: Based on review of staff records the center did not have all staff trained in First Aid and CPR within 30 days of hire. Evidence: On 10/25/27, staff #2 , #3, #4, and #5 did not have certification of First aid and CPR within 30 days of hire. Plan of Correction: Per Director, a First Aid and CPR class is scheduled for Nov. 11th, 2017 for staff who need the training.
