Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Disclosure Statement for new staff hired after 7/1/2017 and requesting out of state Central Registry checks for staff who have lived outside the State of VA within the last five years. The model SDS form is online at VA DSS, a link to the Adam Walsh site to assist with out of state central registry forms as well as contacting VA State Police to research a submitted CRC. VDSS will be paying for initial child care fingerprint checks until 9/20/18. Please read and review all correspondences regarding the fingerprint roll out. Shelter-in place location needs to be clearly designated on emergency map.

Comments:
An unannounced monitoring visit was conducted from 10:20-12:00. Nine children were present with two caregivers (staff member and co-op parent). No medications were reported to be on-site. Children were observed transitioning from snack (food bought from home), hand washing and bathroom breaks, reading and circle time and transition to the outside playground before the session ends. Room is well maintained and equipped with age appropriate materials for preschool children. Emergency drills, first aid kit and non-emergency supplies are up to date. Attendance was current and lunch boxes were dated and labeled. Areas of non-compliance are identified in the violation notice. Description: Based on record review, 1 of 4 children's records was incomplete. Evidence: Child #1 was missing prrof of identity within 7 days of attendance. (date entered 9/6/2017)

Plan of Correction: Center Director will contact the Arlington Parks and Recreation registration office and obtain the required information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, 1 staff record is incomplete. Evidence: Staff #1 did not have documentation of an emergency contact.

Plan of Correction: An emergency contact will be kept on-site for all staff.

Standard #: 22VAC40-185-530-A
Description: Based on staff statement, it was not determined if a staff member was currently certified in first aid and CPR. Evidence: Staff #1 did not have her First Aid and CPR certification card on-site to verify the trainings are current.

Plan of Correction: A copy of the cards will be made and kept on-site.

Description: Based on observation, the most recent inspection findings were not posted. Evidence: The posted inspection findings were dated 7/25/2016 and the most recent inspection was 12/21/2016.
