An unannounced monitoring inspection was conducted on October 26, 2017. There were 16 children present with two staff members. The children were observed playing in centers. The licensing inspector reviewed the physical plant, administration, required postings and documentation, health and safety supplies and procedures, nutrition and supervision. Two staff records were reviewed. Five children's records were reviewed. There are currently no medications at the center. The outdoor playground equipment is not being utilized at this time; the facility is using the open grassy area as the outdoor play space. Exit interview was conducted with the Program director/Lead Teacher.
Standard #: 22VAC40-185-60-A Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on October 26, 2017, the following information was not documented in each child's record: Child #2-Work phone number for one parent Child #5-Home phone number for both parents Plan of Correction: When the parents come to pick up the children, we will have them to correct the files.
