The licensing inspector conducted an unannounced monitoring inspection from 11:40 a.m. until 2:00 p.m. There were 14 children in care with 3 staff supervising. The children were observed during lunch, at circle time, having a bathroom break and preparing for naps. Three staff and five children?s records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-140-A Description: Based on record review, each child did not have a physical examination on record. Evidence: 1. The inspector reviewed five children's records. One of five records (#2) did not have a physical and the date of enrollment had been longer than 30 days. Plan of Correction: The teacher will get the physical from the parents.
Standard #: 22VAC40-185-330-B Description: Based on observation and measurement, the center did not have the required resilient surfacing. Evidence: 1. The inspector observed grass in the fall zone of the single slide on the playground. 2. Measurement of mulch was 3-4 inches under the yellow climber and under the 3 foot tall platforms. Plan of Correction: Center staff will redistribute mulch to the required depth. The center is getting a new playground soon.
