Comments:
54 Children were in care during the SHSI inspection, that was conducted with the director. Children were observed during free play and outside time. The age range of the children was six months to ten years old. Inspection time 2:30pm -5:000pm.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on review of medications for children with food allergies, not all had a allergy plan. Evidence: Child A with food allergy and medication stored at the center did not have a written allergy plan.

Plan of Correction: Returned medication to parent until allergy plan is obtained.

Standard #: VENDSUB-000-031-B-3
Description: Based on review the emergency facility containment did not include all required procedures. Evidence: there was no written procedures for reuniting of children with parents.

Plan of Correction: Create reunification policy and keep in the emergency evacuation file.
