Comments:
An unannounced monitoring inspection was conducted today from approximately 10:40am-12:20pm. There were 83 children in care directly supervised by 12 caregivers. A sample of 8 children?s and 6 staff records were reviewed. The children were observed painting, playing on the playground, watching a movie, reading books, drawing and singing songs. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on records reviewed, children's records were incomplete.

Evidence:
7 of 8 records reviewed were incomplete.
1) Child A was missing all required information.
2) Child B was missing the employer and employer phone number of the mother
3) Child D was missing 1st day of attendance, Immunization and physical.
4) Child E was missing the mother's employer and employer's phone number.
5) Child F was missing the 1st day of attendance.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) Classroom 1, cleaning materials a were unlocked in a cabinet above the sink and on a shelf in the bathroom. 2) Classroom 2, laundry detergent, all purpose wipes and metal polish were unlocked in a cabinet above the bookshelf. 3) Classroom 3, all purpose cleaning wipes were found in an unlocked closet where children keep their shoes and jackets. 4) Classroom 4, cleaning materials were found in an unlocked cabinet above the bookshelf. 5) Classroom 5, cleaning materials were found in an unlocked cabinet above the sink.

Plan of Correction: We will install locks on all cabinets containing these materials in all classrooms.

Description: Based on observation, medication was not kept in a locked place. Evidence: Medication was located in an unlocked cabinet in the "Office".

Plan of Correction: A lock will be intalled

Description: Based on observation, food brought from home was not labeled. Evidence: The lunch boxes of 30 children in care were not labeled with a date.

Plan of Correction: Labels were made for tomorrow's lunch boxes.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and interview with the director, the findings of the most recent inspection of the facility was not posted. Evidence: There were no previous violation notices posted. The director stated that they were kept in a folder.

Plan of Correction: I will post the new one.
