License Inspector spoke with the director about making an appointment for future fire inspections.
An unannounced monitoring inspection was conducted today from approximately 10am-11:50am. There were 58children in care supervised by 12 staff. A sample of 5 children?s records, 3 staff records and 3 medications were reviewed. The children were observed during a nature walk, playing on the playground, crafting a squirrel, sharing in show and tell, reading a book and doing letter association. 2 new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Standard #: 22VAC40-185-60-A Description: Based on records reviewed, children's records were found to be incomplete. Evidence: 2 of 5 records reviewed were incomplete 1) Child A was missing father name, address, phone number, employer and employer phone number. 2) Child B was missing the employer and employer phone number of the mother and father Plan of Correction: Following up with these parents.
Standard #: 22VAC40-185-80-A Description: Based on records reviewed and an interview, it was found that a written record of children in attendance was not maintained for each day. Evidence: Daily attendance for children in care (Room B-8) was not kept for the following days; 9/20/17, 9/21/17, 9/26/17, 9/27/17, 9/28/17, 10/03/17, 10/04/17, 10/05/17, 10/10/17, 10/11/17, 10/12/17. Plan of Correction: I will discuss this with the teachers in that room.
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not kept in a locked place. Evidence: Cleaning materials were found in an art closet that was unlocked in room B12. Plan of Correction: Cleaning materials have been locked.
Standard #: 22VAC40-185-550-M Description: Based on records reviewed, the records of recorded injuries were found to be incomplete. Evidence: Of 9 injury records reviewed: 3 were missing the treatment, 1 was missing the future action, and 6 were missing the time and date that the parent was notified. Plan of Correction: Will note these things specifically in the future.
Standard #: 22VAC40-185-560-G Description: Based on observation, food brought from home was not clearly dated. Evidence: 1) 11 lunch boxes in room B12 were not dated. 2) 9 lunch boxes in room B11 were not dated. Plan of Correction: We will write the date on the name tag attached the lunch box.
Standard #: 22VAC40-191-40-D-1-C Description: Based on records reviewed, the center did not maintain a current background check for each employee. Evidence: More than 3 years has passed since Staff A's Criminal History Check dated 09/2014 had been requested. Plan of Correction: I am going to print the form right now.
