Comments:
An unannounced monitoring inspection was conducted today from approximately 10:10am-12:25pm. There were 90 children in care directly supervised by 24 staff members. A sample of 9 children?s and 5 staff records were reviewed. The children were observed carving pumpkins, making painting, coloring, on the playground and reading with the teacher. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the program director. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on records reviewed, children's records were incomplete. Evidence: 3 of 9 children's records reviewed were incomplete. 1) Child A was missing the work phone number of the father. 2) Child B was missing the addresses of 2 of 2 emergency contacts. 3) Child C was missing the mother employer and employer phone number.

Plan of Correction: Amending registration form to include repopulating each field. Staff will follow up if incomplete.

Standard #: 22VAC40-185-80-A
Description: Based on observation, a written record of daily attendance was not kept. Evidence: 1) In room 125 (3 Day 3's), the attendance was not kept for the following days in the month of October: 4,6,11,13,16,18,20,23,25,27 2) In room 131 (3 Day 3's), the attendance was not kept for the following days in the month of October: 2,4,11,12,13,16,18,20,23,24,25,27.

Plan of Correction: A note was added to the bottom of the attendance sheets to remind teachers to fill out attendance daily.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place. Evidence: 1) In room 112 (Toddler 1) there was an all purpose cleaner on the counter near the sink. 2) In room 126 (Music Room), the first cabinet near the door was unlocked and contained all purpose cleaning wipes, Advil, and disinfectant spray 3) In room 126 (Music Room), the second cabinet near the entrance was unlocked and contained cleaning materials.

Plan of Correction: Cabinets were locked during inspection.

Description: Based on observation, an unused electrical outlet did not have a protective cover. Evidence: An unused outlet on the wall near the cubbies in room 121 (3 day four's) did not have a protective cover.

Plan of Correction: Outlet cover has been replaced.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food brought from home was not labeled. Evidence: 21 lunch boxes were not labeled with the date.

Plan of Correction: I will include that they must label lunch boxes in the newsletter.
