An unannounced monitoring inspection was conducted on October 27, 2017. There were 35 children present at arrival to the facility with four staff members. An additional staff person was on site conducting PALS testing. The children were observed having breakfast which consisted of cereal, milk, and fruit cocktail; washing their hands, brushing their teeth, participating in circle and center time and during music and movement. One medication was reviewed in the facility on this date. Five children's records were reviewed.
Standard #: 22VAC40-185-140-A Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: Before the child's attendance; or within one month after attendance. Evidence: During the inspection conducted on October 27, 2017, there was no documentation of a physical examination in the record for Child #2 (start date 9/5/17) and Child #3 (start date 9/5/17). Plan of Correction: Lead Teacher will call the Health Coordinator to ask for a copy of the physicals.
Standard #: 22VAC40-185-60-A Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on October 27, 2017, the following information was not documented in each child's record: Child #1-place of employment and work phone number for one parent Child #4-complete address for one person to contact in the event of an emergency where the parent cannot be reached Plan of Correction: Lead Teacher will contact the parents for the information.
Standard #: 22VAC40-185-510-E Description: Based on observation and inspection of the facility, the licensee did not ensure that medication was appropriately labeled. Evidence: During the inspection conducted on October 27, 2017, the medication reviewed was not labeled with the child's name. Plan of Correction: Medication was labeled during the inspection.
