Comments:
A Subsidy Health and Safety inspection was conducted on 10/26/2017 to review supplemental health and safety requirements. The inspection started at 9:50 AM and ending at 1:30 PM. Six children and five staff records were reviewed during the inspection. Six medications were reviewed. Lunch and nap were observed and a physical plant inspection was completed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on review of the emergency plan, the center failed to ensure that all items required were apart of the plan. Evidence: 1. The portion on how the center would reunite the children with parents or guardians was not developed.

Plan of Correction: The program director will develop the reunification portion to the plan.
