An unannounced monitoring inspection was conducted on October 20th 2017 between 2:30pm and 5:15pm. There were 82 children in the direct care of 11 staff members. Children were observed eating snack, playing outside, being fed, being diapered, washing their hands, and reading stories. The center was observed to be within ratio compliance. The physical plant was clean and organized and the center offered plenty of supplies for the children to utilize. Violations are listed on the violation notice page. Standard #: 22VAC40-185-130-B Description: Based on record review 1 of 7 children's records reviewed did not have documentation of an updated immunization record every six months for children under the age of 2 years. Evidence: Child #1, born 11/9/2016, last documented immunization update is dated 1/11/2017 Plan of Correction: Parents will supply updated immunization record :Correction 10/23/2017
Standard #: 22VAC40-185-260-A Description: Based on observation, the center does not have documentation to prove that an annual fire inspection was conducted. Evidence: The last fire inspection report on file in the Licensing binder is dated 8/26/2016. Plan of Correction: Fire permit does not expire until 7/31/2018. Center reached out to Fire Marshall office to request an additional inspection. Correction: 10/23/2017
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not kept in a locked place using a safe locking method. Evidence: In the K-Tech classroom, the cleaning cabinet latch was not secured in place to ensure the cabinet was locked. Plan of Correction: Cleaning materials was in a shut cabinet; out of reach of children. Reviewed with staff proper storage of all cleaning materials. Correction: 10/20/2017
Standard #: 22VAC40-185-500-B Description: Based on observation, a diapering surface is not nonabsorbent. Evidence: In the older infant classroom the diapering surface has a tear in one of the corners making the diaper pad absorbent. Plan of Correction: A new diaper pad was ordered. Correction 10/20/2017
