- Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Disclosure Statement for new staff hired after 7/1/2017 and requesting out of state Central Registry checks for staff who have lived outside the State of VA within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out of state central registry forms. VDSS will be paying for initial child care fingerprint checks until 9/20/18. Please read and review all correspondences regarding the fingerprint roll out.
An unannounced monitoring visit was conducted from 10:15-12:30. All children were in ratio with direct care staff. Observed children participating in infant care, free play outside, diapering areas, an art project and transition from AM Kindergarten. Rooms are well maintained, organized and equipped with age appropriate materials for children. A sample of staff and children's records were reviewed, emergency preparation, trainings, medication authorizations and storage as well as nutrition. Lunch served today was cheese pizza, salad, pineapple and milk. Infant bottles were labeled and dated. Two staff have Medication Administration Training (MAT) certification with emergency medications on-site. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 3 children's records is incomplete. Evidence: Child #1 does not have an action plan for a documented food allergy. Plan of Correction: Center Director will contact the parent to obtain an action plan.
Standard #: 22VAC40-185-530-A Description: Based on staff statement, children who are on the center bus are not in care of a staff member who has current first aid and CPR certification. Evidence: Children who ride the bus with the current bus driver are not in care of a staff member with current first aid and CPR certification. The bus driver has been the only staff member on the bus since September 2017 Plan of Correction: Staff #1 is scheduled to take a first aid and CPR course on 11/4/2017.
Standard #: 22VAC40-185-580-L Description: Based on record review and staff statement, no parental permission to transport children has been secured. Evidence: Children are dropped of and picked up from the local elementary school for the school-age program. Children's records do not include written parent permission for the center to transport. Plan of Correction: Center management will obtain parent permission for all children who ride the bus on regularly scheduled trips to the elementary school.
