22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
The licensing inspector conducted an unannounced complaint inspection on 10/30/17 in response to a complaint that was received in the licensing office on 10/11/17 regarding an allegation in the area of Special Care Provisions. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegation, so the complaint is determined to be VALID.
Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on a record review and interview with staff, the center failed to maintain a written record of children's minor injuries in which entries are made the day of occurrence. Evidence: Child #1 who enrolled in the center on 08/07/17 and disenrolled on 10/06/17, was bitten twice by child #2 on the arm. Staff #1 and Staff #2 both confirmed the child #1 was bitten by child #2 twice during his enrollment in the center. The director was unable to provide documentation of when these bites occurred. Child #1 had 5 other incident reports in his file for minor injuries, however none of these reports included being bitten by child #2. Plan of Correction: The staff was retrained on incident reporting on October 17, 2017. All incident reports and observations are to be reported to the management staff immediately or as soon as it is practicable. The injury log will be managed daily by the management team to ensure proper documentation and communication. DM will inspect periodically.
