Standard #: 22VAC40-185-550-M
Complaint related: Yes
Description: Based on a record review and interview with staff, the center failed to maintain a written record of children's minor injuries in which entries are made the day of occurrence. Evidence: Child #1 who enrolled in the center on 08/07/17 and disenrolled on 10/06/17, was bitten twice by child #2 on the arm. Staff #1 and Staff #2 both confirmed the child #1 was bitten by child #2 twice during his enrollment in the center. The director was unable to provide documentation of when these bites occurred. Child #1 had 5 other incident reports in his file for minor injuries, however none of these reports included being bitten by child #2.

Plan of Correction: The staff was retrained on incident reporting on October 17, 2017. All incident reports and observations are to be reported to the management staff immediately or as soon as it is practicable. The injury log will be managed daily by the management team to ensure proper documentation and communication. DM will inspect periodically.
