- Technical Assistance:
Inspector spoke with the Director about 22VAC40-185-70 during this visit.
An unannounced monitoring visit and facility tour was conducted from 10:08am- 11:05 am. At the time of inspectors' arrival there were 20 pre-school and school aged children in care with 4 staff members and 1 administrator. Children were observed participating in indoor play, nap, snack, transitioning between activities, and hand washing. A sample of 5 children's records and 5 staff on-site records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.
Standard #: 22VAC40-185-260-B Description: Based on record review it was determined that the center could not provide annual approval from the health department. Evidence: The center did not have documentation of a current health department inspection. Plan of Correction: Director will contact the health department to schedule a health inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: In the outdoor play area there was a large plastic bin that was cracked presenting potential injury hazards. There were also three tricycles with exposed rust on the handlebars. Plan of Correction: The center will replace and/or repair the equipment listed above.
Standard #: 22VAC40-185-550-M Description: Based on record review it was determined that the center did not ensure that the written record of children's serious and minor injuries contained all of the required components. Evidence: The injury reports dated 9/26/17 for child #6 and 9/25/17 for child #7 did not contain the required future action to prevent recurrence. Plan of Correction: Director will ensure that all staff utilize the correct injury report form that contains all of the required components.
