22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from approximately 9:45am-12:50pm. There were 67 children in care directly supervised by 14 staff. A sample of 7 children?s and 5 staff records were reviewed. The children were observed playing on the playground, coloring, making aqua bottles and painting. Zero new staff has been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the assistant director. Standard #: 22VAC40-185-60-A Description: Based on observation, children's files were incomplete. Evidence: 5 of 7 files were found to be incomplete. 1) Child A was missing a signature for communicable disease notification, signature on part 1 of the health form, proof of Identification. 2) Child B was missing signature on part 1 of the health form. 3) Child C was missing work address of male guardian (uncle). 4) Child D was missing date of enrollment. 5) Child E was missing physical health exam and health care information. Plan of Correction: Request information from parents to be returned as soon as possible.
Standard #: 22VAC40-185-270-A Description: Based on observation, the building was not maintained in an operable condition. Evidence: The gate on the playground leading to the infant play area requires a bungee cord to remain shut. Plan of Correction: Submit a work order to have that fixed.
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not kept in a locked place. Evidence: In the school age room, there were cleaning materials unlocked in a cabinet above the sink. Plan of Correction: Corrected during inspection.
