The licensing inspector conducted an unannounced monitoring inspection Tuesday, November 7, 2017 from approximately 10:50AM to 1:15PM. The following ratios were observed with a total of 28 children present during the inspection: Toddlers - 5 children with 1 staff Twos Room - 5 children with 2 staff Threes Room - 9 children with 2 staff Fours and Fives Room - 9 children with 1 staff The children were observed engaged in a variety of activities during the inspection. Children were observed working puzzles, dancing to music, reviewing the letter ?Hh? and words that started with ?Hh?, reviewing animal sounds and preparing for lunch. Snack is provided and children bring their lunches from home. Later children were observed resting at nap time. All areas of the facility including classrooms, hallways, kitchen, and bathrooms were inspected. Playgrounds were not inspected due to inclement weather. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily attendance, emergency supplies, required postings, practiced emergency drill logs, staff training information and parent agreements. The provider stated medication is not administered at this time. Transportation is not provided. Standard #: 22VAC40-185-280-G Description: Based on observation, the facility did not ensure all substitute containers were clearly labeled with their contents. Evidence: During the inspection, the licensing inspector observed two unlabeled spray bottles containing a liquid by the diaper changing area. A staff member stated the spray bottles contained soap and water and bleach water. Hazardous substances not kept in original containers are required to be clearly labeled with the contents. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-520-C Description: Based on observation, the facility did not ensure written parent authorizations were present for all of the children's over the counter ointments. Evidence: During the inspection, the licensing inspector observed a container of Vaseline present for Child #3 present in the Toddler room. An authorization form could not be located while the licensing inspector was one site. Plan of Correction: We will have the parent complete another authorization form today.
