An unannounced Subsidy Health and Safety Inspection was conducted on October 25, 2017 from approximately 10:40 a.m. until 3:00 p.m. Upon entrance there were 22 children in care and 5 staff present with children. The record review sample consisted of 5 children's records and 5 staff records. Children were observed eating lunch, preparing for and during rest period and playing in centers to include blocks and imagination. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Standard #: VENDSUB-000-004-F Description: Based on observation and interviews, the vendor did not ensure to post with parental approval, or kept in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: During the inspection on October 25, 2017, a list of 3 children and their food sensitivities (whole milk) was posted in four classrooms in the facility and parental approval had not been obtained Plan of Correction: The center director will get written permission to post.
Standard #: VENDSUB-000-012-E Description: Based on observation and interviews, the vendor did not ensure all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval CPR and first aid training. Evidence: During the inspection on October 25, 2017, staff #6 did not have documentation of obtaining first aid training. The center director confirmed staff #6 began working in 2016. Plan of Correction: The center director will have staff member sign up and take the class.
Standard #: VENDSUB-000-031-B-1 Description: Based on documentation review and staff interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an evacuation, shelter-in- place and lockdown. Evidence: During the inspection on October 25,2017, there were not reunification procedures documented for evacuation, shelter-in-place and lockdown. Plan of Correction: The center director will write out the reunified plan and add it to the emergency procedures.
