22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 10:50am-2:00pm. There were 30 children (infants-12yrs.) directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, doing puzzles, eating lunch and transitioning into naptime. Lunch consisted today of beef macaroni with tomato sauce, broccoli and pineapples. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-B Description: Based on record review, a child under the age of two did not have documentation of additional immunizations updated once every six months. Evidence: Child#2 who is currently under two years old (DOB: 6/2/16), has an immunization record on file that was last updated on 10/6/16. Plan of Correction: An update of immunization records will be requested.
Standard #: 22VAC40-185-500-B Description: Based on observation and staff interview, a nonabsorbent surface was not being used at the center for diapering. Evidence: A nonabsorbent surface for diapering was not being used in the toddlers classroom, the changing mat was covered by a cloth cover which is not considered to be nonabsorbent. Plan of Correction: Fixed during the inspection.
Standard #: 22VAC40-185-510-C Description: Based on record review, a child has a long term medication authorization form on file that has expired. Evidence: Child#1 has a medication authorization form for a prescription medication that expired on 3/8/17. Plan of Correction: A new authorization form will be requested.
