22VAC40-191 Background Checks (22VAC40-191)

Discussion was held on the topics of current enrollment, current staffing, the new definition of a serious injury, and a review of changes to background checks with regards to expiration dates, new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. Discussion of propping crib mattresses up. Please, use a standard wedge and follow the manufactures instructions. Please, obtain a note from the child's parent giving the center permission. A doctor's note may also be needing due to the child's health. Further research shall be obtained and given to center.
The following inspection took place between the hours of 10 am through 2 pm. Today, I met with the Staff of Your Child's Place Child Day Center located in Vienna, Virginia. At the start of this unannounced monitoring inspection there were a total of 62 children with 12 staff; good ratios in both buildings; all 7 rooms. Please see violation notice for ratio on playground. A total of 5 children?s files and 6 staff files were reviewed. The children were engaged in the following: large motor skills on the playground, story time, learning activities, art, games, infant feeding, diapering, bathroom breaks, hand washing, lunch time, nap time, and other organized activities. Standard #: 22VAC40-185-270-A Description: Based on observation, one area within the Child Day Center (CDC) was maintained in an unsafe condition. Evidence - 1. On the date of inspection (11/01/2017), peeling paint was observed within the downstairs "Flower 1" room. 2. The spot where paint have peeled off was observed in the diapering area. Peelings were observed on floor as well. 3. A total of three 2 year old children were observed in the classroom at the time of observation. Plan of Correction: Maintenance shall correct the peeling area. All peelings that had fallen to the floor were removed on the date of inspection.
Standard #: 22VAC40-185-340-A Description: Based on observation, staff did not ensure protection of the children in care by having a blind cord that hung low enough that it became accessible. Evidence - 1. On the date of inspection (11/01/2017), a blind cord within the Bumblebee's room for the window on the left wall of the classroom was not tied in place. 2. On the date of inspection, there were a total of 9 children within this class at the time of observation. Plan of Correction: Corrected on the date of inspection. The cord was tied up.
Standard #: 22VAC40-185-350-C Description: Based on observation, the staff-to-children ratio based on the youngest child in a mixed age group was not met. Evidence - 1. On the date of inspection (11/01/2017) at approximately 11:30 am, a mixed group of toddlers & 2's were observed on the center playground. 2. The group consisted of a total of 13 children with 2 center staff. The youngest child was between 18 to 24 months. Therefore, the ratio for this mixed group should have been 1 staff person for every 5 children. 3. The group remained out of ratio until another staff person arrived shortly thereafter. Prior to the third staff's arrival it was undetermined how long this group functioned out of ratio. Plan of Correction: Corrected on the date of inspection.
Standard #: 22VAC40-185-510-G Description: Based on review and interview, the center has on site medication that was dispensed from a pharmacy, but not maintained in the original, labeled container. Evidence - 1. On the date of inspection (11/01/2017), medication prescribed for Child #7 was not stored in its original container. 2. The Center Director stated (during an inspection conducted on the date of inspection) that the parent had told them that the original container was discarded prior to the center receiving it. Plan of Correction: The current medication is set to expire at the end of March 2018. The Parent will be reminded to bring in the new refilled medication with the original box prior to that time.
