Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted November 9, 2017. The on-site inspection was from 2:30 p.m. to 6:00 p.m. On this date, there were 193 children in six groups and in the care of 15 staff members with one staff member to assist if needed. The total enrollment for this week's program was 241 children. There were 10 staff members' records and 10 children's records reviewed . The medication and medication paperwork were reviewed for five children. There were eight staff members and six children interviewed. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were documented on the supplemental page. Some items reviewed and observed were: playing in the Big Room, working on homework, playing large motor games in the Arena, bathroom and hand washing procedures, transitions, snack, table top games, art activities, one first aid kit, required posted information, injury reports, updated emergency evacuation plan, documentation of evacuation and shelter-in-place drills, and dismissal procedures. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of ten staff records and verification from the director, one new staff member failed to obtain a negative tuberculosis (TB) screening within 21 days of employment. Evidence: 1. The record of Staff Member #5 contained documentation that a negative TB screening was completed on 01/27/2017 which was not within 21 days of employment. The date of hire for Staff Member #5 was 01/03/2017. 2. The director confirmed the TB screening had not been obtained within 21 days.

Plan of Correction: The director will ensure that all TB screenings are completed within 21 days of hire.

Standard #: 22VAC40-185-160-C
Description: Based on review of ten staff records and confirmation from the director, one staff member failed to obtain a negative tuberculosis (TB) screening within two years of the date of the most recent screening. Evidence: 1. The record of Staff Member #6 contained documentation that a negative TB screening was completed on 06/07/2017and the previous screening was 09/22/2014. 2. The director confirmed there was more than two years between the TB screenings.

Plan of Correction: The director will ensure that TB screenings are completed at least every two years from the date of the most recent screening.
