Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on November 7, 2017 from approximately 1:05 p.m. until 3:30 p.m. Upon entrance there were 25 children and 4 staff present with children. The record review sample consisted of 5 children's records and 4 staff records. Children were observed playing board games and large group games. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Description: Based on observation, the vendor did not ensure, there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on November 7, 2017, there are two children with diagnosed food allergies and there is not a physicians plan available for review, child #2 (fish and peanuts) and child #6 (eggs).

Plan of Correction: The center director will work with the supervisor to get the action plans from the parents.
