Comments:
An unannounced monitoring inspection was conducted today from 10:00am-11:15am. At the time of inspection, 33 children (infants through five years) were directly supervised by 7 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Medications and required forms were inspected. Children were observed working together to move a parachute, engaged in group play outdoors, and infants were observed having tummy time. Staff interactions with children were positive and one-on-one guidance was observed. Child artwork and projects were displayed throughout the center with pictures of the children completing the many activities. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-A
Description: Based on a review of records, 1 of 5 child records did not have documentation of immunizations before the child could attend the center. Evidence: Child #1 (start date: October 2017) did not have immunizations on file at the child day center.

Plan of Correction: Child #1 parent was called and immunization record will be given 11/9/17.

Standard #: 22VAC40-185-510-D
Description: Based on a review of medication records, 2 of 8 child medications did not have medication authorization forms. Evidence: Child #2 and Child #3 had written allergy plans for the prescription medications on site; however, they were missing the required medication authorization forms.

Plan of Correction: Forms for medication authorization were given to both families to be filled out.
