Comments:
An unannounced monitoring inspection was completed on November 9, 2017. The purpose of this visit was to measure compliance with the licensing standards. There were 15 children, toddler to preschool age, in care at the time of the inspection. There were 3 staff members working directly with the children in their classrooms. All classrooms maintained the proper ratios. The children throughout the facility were engaged in activity. The toddlers and twos were reviewing letters and using animal letter cards. The preschool class was saying nursery rhymes while waiting for lunch to be delivered. The children ate macaroni and cheese, bread, bananas, broccoli and milk for lunch. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways and bathrooms were inspected. The playground was not inspected due to rainy weather conditions. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Medication is administered and the director has MAT training. Multiple staff have cpr and first aid training. Last documented health inspection: 5/10/17 Last documented fire inspection: 10/12/17 During the inspection, 5 children's records and five staff records were reviewed. Approximate time on site: 11:00am to 1:45pm

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on a review of a sample of staff records, the facility did not ensure that a staff member completed 16 hours of annual training. EVIDENCE: The record of staff person # 4, start date 11/7/16, contained documentation of only 13.25 hours of annual training.

Plan of Correction: The staff person will complete the additional hours by today.

Standard #: 22VAC40-185-270-A
Description: Based on observations in the preschool/school age building. the facility did not ensure that the areas and equipment of the center were maintained in a safe and operable condition. EVIDENCE: The inspector observed a storage cabinet in the after school space that was wobbly and unstable. The inspector also observed two holes in the partition wall in the after school space.

Plan of Correction: The cabinet was removed today and the holes were repaired on site.
