Comments:
An unannounced complaint inspection in response to a complaint that was received by the licensing office on 09/05/2017. Interviews were conducted with staff relating to allegations of child to staff ratio, building and grounds and medications. The information gathered during the investigation supports the allegations, so the complaint is determined to be valid.

Description: Based on observations on 9/7/2017 of the toddler/two year old room, the center failed to ensure that all children were with in sight and sound supervision during rest time. Evidence: 1. Three staff were present in the joined classroom during rest time. Three two year old children were resting behind a book shelf and were not able to be seen by the staff. The book shelves were a divider between the toddler's and the two year old classroom.

Plan of Correction: The staff person that belonged in the two year classroom moved over to that area once reminded. Director will monitor to ensure that staff is positioned correctly.

Description: Based on observation of the infant room on 10/24/2107, the center failed to ensure that the infant to staff ratio was met. Evidence: 1. Observed nine infants from 4 months to 12 months with two staff members. The required staff was three staff for nine infants. The ratio is one staff for four infants.

Plan of Correction: The director will ensure that additional staff will be placed in the infant room that does not include the staff person in the second infant room.

Description: Based on interviews with staff, the center failed to ensure that all medications were locked.

Evidence:
1 A pill was found in child # 1 hand. The director took the pill to the physician's office to verify the type of pill. The physician's office recommended calling Poison Control. The director and staff could not verify that the child ingested the pill. The child showed no evidence of taking any medications. The staff are not sure on where the pill originated.

Plan of Correction: The director and board president developed a new policy that diaper bags are to only have supplies that can be used at the center from this point on.
