Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Wednesday, November 1, 2017 from approximately 10:10AM to 3:00PM. The following ratios were observed with a total of 113 children present during the inspection:
Young Infants - 8 children with 2 staff
Middle Age Infants - 7 children with 2 staff
Older Infants (Red Room) - 7 children with 2 staff
Transitional Toddlers (Red Room) - 10 children with 2 staff
Twos (Orange Room) - 10 children with 1 staff
Twos (Yellow Room) - 15 children with 2 staff
Twos (Green Room) - 17 children with 2 staff
Threes (Blue Room) - 9 children with 1 staff
Fours (Purple Room) - 10 children with 1 staff
Fours & Fives (Gold Room) - 19 children with 1 staff
Get Well - 1 child with 1 staff

The children and staff were engaged in a variety of activities when the licensing inspector arrived. Infants were observed playing on the floor with their teachers, having their diapers changed, eating and sleeping in their cribs. The Toddlers were observed looking at books, eating lunch, having their diapers changed and resting at naptime. The Twos and Threes classrooms were reviewing shapes and colors in Spanish and English, having books read, eating lunch, playing outside and resting at nap time. The Fours and Fives were observed painting, playing in home living, building with blocks, playing musical instruments, playing in the sensory table and eating lunch. Lunch consisted of Sheppard?s Pie, peas, carrots, fruit and milk. All areas of the center including classrooms, hallways, kitchen, playgrounds, and bathrooms were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, daily sheets, daily attendance, infant feeding schedules, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered, and medication and authorization forms were reviewed.

Standard #: 22VAC40-185-510-G
Description: Based on review of eight medications, the facility had one prescription medication present at the facility without a pharmacy label. Evidence: During the inspection, the licensing inspector observed a container of prescribed eye drops present at the facility for Child #9. The prescribed eye drops did not have a pharmacy label on the bottle. A pharmacy label for the prescribed eye drops could not be located while the licensing inspector was on site.

Plan of Correction: Medication was returned to parents that day. Our Nurse Coordinator retrained our part-time nurse to ensure correct medication labeling. Preschool Director will check meds daily.

Standard #: 22VAC40-185-520-A
Description: Based on observation, the facility did not ensure over-the-counter skin products were not kept or used beyond the expiration date of product. Evidence: During the inspection, the licensing inspector observed an over-the-counter skin product for Child #4 present in the classroom. The expiration date for the skin product was stamped as expiring in August 2017.

Plan of Correction: Diaper creams was discarded at visit. Diaper cream dates will be check on the 1st of each month by the Lead Teacher and spot checked by School Nurse.
