Comments:
An unannounced monitoring inspection was conducted on 11/02/2017 from 12:40pm - 2:35pm. At the time of entrance there were 64 children, ages infant - five years, in care with 11 staff. Children were observed during nap. Afternoon snack was observed in one preschool classroom. Diapering was observed in the infant class. Records were reviewed for ten children and nine staff. Written Injury reports, medication, emergency supplies and documentation of emergency practice drills additionally reviewed during the inspection. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas of the center shall be maintained in a safe condition. Evidence: There was an area of loose vinyl flooring that had come up in the preschool I boys bathroom. This is a trip hazard.

Plan of Correction: During the inspection the area of loose flooring was taped down using duct tape. A maintenance request will be placed today for permanent repair.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, the center failed to ensure that staff have a central registry finding within 30 days of employment. Evidence: 1. There was no central registry finding on file for staff 1. Staff 1 has a hire date of 09/21/2017. 2. Documentation was on file to indicate that a CPS check had been requested on 09/26/2017 however, there was no documentation to indicate if any follow-up had been conducted to check on the status of the search.

Plan of Correction: A telephone call was placed to CPS on the date of the inspection to check on the status of the search for staff 1. A voice mail message was left. A second call , and VM message, was left with CPS on Monday, November 6, 2017. Administrative staff will follow-up with the inspection when the search is received.
