An unannounced monitoring inspection was conducted from 9:30 am - 12:15pm. There were 84 children in care being supervised by 15 staff members. The children were observed participating in structured center play, Montessori activities, and eating morning snack. Meals are provided from home. Ten children's records were reviewed. Six staff records were reviewed. Nine medications were reviewed. The most recent emergency escape drill was conducted 11/6/17 and the most recent shelter in place drill was conducted 8/10/17. Standard #: 22VAC40-185-140-A Description: Based on review of 10 children's records it was determined that not all records contained documentation of a physical within the first 30 days of enrollment. Evidence: Child F with a start date of 07/05/17 did not have documentation of a physical on file. Plan of Correction: Family has been informed of missing health forms on 11/9/17, awaiting doctor's office fax of last well check.
Standard #: 22VAC40-185-60-A Description: Based on review of ten children's records it was determined that not all records were complete. Evidence: The records for Child C and Child E did not name two emergency contacts on file. The record of Child D did not have the addresses listed for two emergency contacts listed on file. Plan of Correction: Families have been informed and 2 addresses have been provided to the school.
Standard #: 22VAC40-185-510-A Description: Based on observation of nine medications it was determined that not all medications were accompanied with a parental authorization. Evidence: Medication for Child B was not accompanied by a medication authorization form. Plan of Correction: Parents filled in 10 day medical authorization form and have a form to turn into their pediatrician for a long term authorization.
Standard #: 22VAC40-185-510-E Description: Based on observation of nine medications it was determined that not all medications were labeled with the child's name, dosage, and time to be given. Evidence: The emergency medication for child A did not accompany a prescription label to identify the name, dosage, and time to be given. Plan of Correction: Parent brought in prescription label.
Standard #: 22VAC40-191-40-D-1-C Description: Based on review of six staff records it was determined that not all criminal history checks were repeated every three years. Evidence: The most recent criminal history check on file for Staff B was dated 06/26/14. Plan of Correction: Form SP230 was printed and mailed on 11/13/17.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of six staff records it was determined that not all employee's were denied continued employment when the criminal history check was not obtained within the first 30 days of employment. Evidence: Staff A with a start date of 9/6/17 was allowed continued employment without having documentation of a criminal history check within the first 30 days of employment. Plan of Correction: VA State Police background check offices were called on 11/9/17. File was found and processed. Administration was informed from VA State Police on 11/13/17 that the completed from was in the mail to the facility.
