Description: Based on observation, childen's files were incomplete. Evidence: 10 0f 10 files reviewed were missing required information. 1) Child A was missing the address of 1 of 2 emergency contacts. 2) Child B was missing the addresses of 2 of 2 emergency and the mothers employer. 3) Child C was missing the addresses of 2 of 2 emergency contacts an the employer and employer phone number of both parents. 4) Child D was missing the addresses of 2 of 2 emergency contacts. 5) Child E was missing the addresses of 2 of 2 emergency contacts and the employer and employer phone number of the mother. 6) Child F was missing the address of 1 of 2 emergency contacts and all required information for an additional emergency contact. 7) Child G was missing 2 of 2 emergency contacts and the employer and employer phone number of both parents. 8) Child H was missing the address of 1 of 2 emergency contacts, all required information for an additional emergency contact, employer and employer phone number of both parents, and the fathers name, address and phone number. 9) Child I was missing the addresses of 2 of 2 emergency contacts, employer and employer phone number of both parents and fathers name, address and phone number. 10) Child J was missing he addresses of 2 of 2 emergency contacts, employer and employer phone number of both parents and fathers name, address and phone number.

Description: Based on observation, cleaning materials were stored above and on the same level as food. Evidence: 1) Food was on the same level as cleaning material in a locked cabinet near thin sink in Room 3. 2) Food was on the same level as bottle labeled "sanitizer" in locked cabinet next to entrance in Room 4. 3) Food was on the same level as cleaning materials in locked cabinet near entrance in Room 6. 4) Cleaning materials were located above food in locked cabinet near sink labeled "do not use" in Room 8.

Description: Based on observation, electrical outlets did not have protective covers. Evidence: 1) 1 electrical outlet on power strip on top of table near laminator in Room 3 did not have a protective cover. 2) 1 electrical outlet on power strip on the floor near computer desk in Room 3 did not have a protective cover. 3) 1 electrical outlet accessible to children behind cabinet where the radio is located in Room 8 did not have a protective cover.
