Serious Injury Reporting, new applications, and finger printing.

Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, group time, outside time, personal hygiene, lunch time, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. Thirty-two (32) children, aged 4 months-5 years, were found to be in care with 6 staff and office staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9am and concluded at 1:15pm.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of staff files the facility failed to maintain compliance with this standard as required relating to PPD screenings/tests. Evidence: Staff record #3, hire date 10/24/17, did not contain a TB/PPD test or screening; the allowed 21 days to obtain this had expired.

Plan of Correction: This will be obtained immediately and placed in the file. Review of staff files will continue to ensure future compliance.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records the facility failed to maintain compliance with this standard as required relating to references and proof of education. Evidence: Staff record #1, hire date 11/14/17, did not contain references as required. Staff record #3, hire date 10/24/17, had only 1 of the 2 required references. Staff record #3, hire date 10/24/17, did not have proof of education/qualifications.

Plan of Correction: The required information will be obtained and placed within the files as soon as possible. File reviews will continue to ensure future compliance.

Standard #: 22VAC40-185-330-B
Description: Based on review of the playgrounds the facility failed to maintain compliance with this standard as required related to resilient surfacing and slide zone requirements. Evidence: The newly installed playground equipment for the toddler playground did not have resilient surfacing under and around the fall zone as required. The newly installed playground equipment for the younger preschool children did not have enough slide zone exit, for the taller slide, as required by the ASTM/CPSC playground requirements. The requirement is 7'3", and currently there is 7'. The requirement is 6'7", currently there is 4'.

Plan of Correction: These areas will be addressed and brought into compliance as required. Maintenance will be notified with a work request as well.
