Comments:
An unannounced monitoring inspection was conducted from 9:30am-1:00pm. At the time of entrance, 20 children were in care of 4 staff members present. Children were observed during storytime, nursery rhymes, and lunch. A selection of 4 staff and 6 children records, medications, injury reports, emergency procedures, and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on a review of records, 1 staff was missing documentation of education, staff development, certification, and experience required by the job position. Evidence: The record for Staff #3 - identified as a lead teacher - was missing transcripts and/or diploma.

Plan of Correction: Staff will bring a copy of qualifications.

Standard #: 22VAC40-185-240-A
Description: Based on a review of records, 1 of 4 staff were missing documentation to demonstrate that staff have received orientation training by the end of their first day of employment. Evidence: The record for Staff #4 [date of hire: 9/30/2017] was missing documentation of orientation training.

Description: Based on review of records and discussion with administrator, no staff currently are trained in daily health observation, Evidence: Daily Health Observation for Staff #5 [not on duty at time of inspection] expired 2/17/2017.

Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method. Evidence: A total of 6 cleansers were observed in 2 unlocked upper cabinets in the infant classroom.

Description: Based on review of medications and children's records, parental authorizations to administer medications were not maintained. Evidence: 1. The medication identified on the written authorization [brand name in the form of a liquid] for Child #4 did not match the medication on site [generic in the form of a tablet]. 2. The written authorization for a medication belonging to Child #6 [observed on site] expired 12/2016.

Description: Based on observation, over-the-counter skin products were kept beyond the expiration date of the product. Evidence: 3 diaper creams were observed on site which had expired in 10/2016, 1/2017 and 2/2017.

Description: Based on observation, diaper creams and ointments were not labeled. Evidence: 4 out of 12 diaper creams were not labeled with children's names.

Description: Based on review of emergency supplies, the first aid kit did not contain the required items. Evidence: The first aid kit was missing 2 of 2 required triangular bandages.

Description: Based on a review of records, 2 staff hired after July 1, 2017 did not complete the new sworn disclosure statement form as required. Evidence: Staff #3 [date of hire August 2017] and Staff #4 [date of hire: 9/30/2017] did not sign the new sworn disclosure statement form.

Plan of Correction: I will have staff complete the updated form.
