Comments:
An unannounced monitoring inspection was conducted today. All rooms were in ratio during the inspection. 5 staff and 5 children's records were reviewed. See violation notice for missing items. You will have 10 days to show documentation of completed out of state background search forms. Children were observed washing their hands, eating breakfast, playing outdoors and enjoying books in the classrooms. Violations:
Standard #: 22VAC40-185-240-D-4
Description: Based upon Director's statement and a review of available documentation, children with emergency medications are not always in the care of a staff person who can administer specific medications. Evidence: There are 2 children present that have Auvi-Q epinephrine medication prescribed to them. There are no staff members that have the specific Auvi-Q training certification.

Plan of Correction: We will get staff trained. I will contact the trainer and see when a training can be arranged.

Standard #: 22VAC40-191-60-B
Description: Based upon a review of staff records, all required background documentation is not on file. Evidence: Staff A does not have a current Sworn Disclosure statement on file. The form used does not include required information regarding any residence outside of the state of Virginia in the past 5 years.

Plan of Correction: Current forms were printed during the inspection. All staff present have completed the current form.

Standard #: 22VAC40-191-60-C-1
Description: Based upon a review of staff records, staff records do not include all required background information, within 30 days of employment. Evidence: Staff A does not have a completed Criminal Record Check (CRC) on file within 30 days of employment. (Date of Hire (DOH):10/10/2017)

Plan of Correction: I have contacted Richmond, to see where the results are.

Standard #: 22VAC40-191-60-C-2
Description: Based upon a review of staff records, staff records do not include all required background information, within 30 days of employment. Evidence: Staff A does not have a completed Central Registry Search (CPS) on file. (DOH: 10/10/2017)

Plan of Correction: I have contacted Richmond, to see where the results are.

Standard #: 63.2-1720.1-B-3
Description: Based upon a review of staff records, records do not contain all required background information. Evidence: Two staff have resided outside of the State of Virginia in the last 5 years. There is no documentation to show that a CPS search has been conducted for the state where the staff persons previously resided. (Staff B - North Carolina, Staff C- Ohio).

Plan of Correction: I will get the forms completed and submitted.
