Technical Assistance:
The agency keeps electronic records for the enrolled children and places a printed enrollment form in each child's physical record on site. Please ensure that if there is information captured electronically that is required in a child's record and that is not present on the hard copy stored on site, that staff can access the information if needed.

Comments:
An unannounced monitoring inspection was conducted from 12:30 p.m. to 1:50 p.m. At the time of entrance the following adult to child ratios were observed: 18 children, ages 3 to 5 years old, with 2 staff and a third adult who was working directly with one child. The records sample size consisted of 5 children?s records. Staff records are housed off site and will be reviewed at a separate inspection. Children were observed resting quietly at nap time and one child was observed interacting with an adult and investigating materials throughout the classroom. Medication administration was not observed. Staff #1 stated that there are no children currently enrolled who are administered medicaton while in care. Children are not routinely transported in a vehicle while in care and staf report no accidents or injures requiring first aid or medical care since the last inspection. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Description: Based on a review of records, the licnesee did not ensure that each enrolled child has a complete record with all required information to include he name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Evidence: Five children's records were reviewed. The record for child #5 did not have an address listed for one of the two required emergency contacts to be contacted in an emergency if a parent cannot be reached.

Plan of Correction: This information will be obtained from the parent today and documented in the child's record.
