Comments:
An unannounced monitoring inspection was conducted on November 07, 2017 approximately from 10:15am -1:00 pm. At the time of entrance there were 45 children and 11 staff members present with children. The sample size consisted of 5 children's records and 5 staff records. The playground was inspected . Children were observed during outside play time, nap time, eating lunch and freely playing. Areas of non-compliance are identified on the violation notice. The exit interview was conducted with the center director. Violation notice for November 7, 2017 was amended on November 20, 2017 and the amended report was e-mail to the licensee on November 20, 2017 for signatures and corrections.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on November 07, 2017, the licensing inspector observed peeling paint on multiple areas of the playground composite.

Plan of Correction: We are working with conjunction of our facilities department to ensure that the chipping paint is repaired on or before 12/15/2017. Safety captain will be responsible for this weekly.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the licensee did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on November 7, 2017, the licensing inspector observed in a unlocked cabinet in the Pre-K classroom there were two bottles of disinfectant.

Plan of Correction: All staff will be trained on the proper storage of hazardous substances on or before 12/15/2017. Management will observe for compliance daily.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure emergency evacuation and shelter-in-place procedures/maps were posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection on November 07, 2017, shelter in place was not marked on the posted map in the building.

Plan of Correction: New evacuation maps will be completed on or before 11/20/2017. Safety captain will review monthly for compliance.
