An unannounced renewal inspection was completed 11/20/17. There were 16 children present in three groupings with three direct care staff supervising and three administrative staff present. Five children's records and five staff records were reviewed. No medications at the center. The inspector arrived at 8:15 am and departed at 1:45 pm. The children were observed during arrivals, during free time in the classrooms, during structured activities, during outside time, during lunch, and during nap. There was discussion with the director and other administrative staff about the following: daily health observation training update needed by April 2018, procedures for new staff during diapering and table preparation for eating, required documentation by the physicians on children's physicals.
Standard #: 22VAC40-185-40-E Description: Based on observation, review of policies and procedures and interview with the director, the center failed to ensure that the center's activities, services, and facilities were maintained in compliance with these standards and the center's own policies and procedures that are required by these standards. Evidence: 1. A required policy to be given to parents in writing for arrival and departure of children was reviewed in the parent handbook (page 10 - Dropping off and picking up students - of the "Parent Handbook" revised February/May 2017). The policy stated that, "Parents are to enter the building, sign the student in, take them to the classroom and make sure that a teacher or appointed staff sees the child and verbal communication between you and the teacher or appointed staff occurs that there is an awareness that the student has arrived." 2. At approximately 8:30 am, a four year old child was observed to come in the center and signed in at the front by the parent. The parent did not walk the child to the classroom or into the area with the other children and staff. The parent did not leave the check in area at the entrance to the center. The distance between the check-in area and the child care area is approximately 50 feet separated by double doors. The child stopped in the hallway and entered a side conference room area where the inspector was working. The staff at the center were unaware that the child was in the room until the inspector notified the director. The child was in the room with the inspector for approximately two to three minutes. 3. At approximately 9:17 am, a parent was observed checking in two children ages two and three years old. The two children walked down the hallway alone. There were no staff directly on the other side of the double doors and the parent did not take the children to the staff or into the center. The parent did not leave the check in area at the entrance to the center. No staff were aware that the children had not been turned over to a staff person. 4. The parents of the children had signed a document (first parent signed in 2016 and the second parent signed in 2017 ) stating that they had read the parent handbook and agreed to abide by the policies. The director stated and verified through past documentation that the 2016 parent handbook had the same drop off and pick up policies/procedures. Plan of Correction: The doors will be locked at opening so a staff person must greet the parents and open the door to send them down the hall with the children to the staff in the classroom.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that two of five children's records contained complete documentation that is required by the Standards. Evidence: 1. The record of child #1 did not have a second emergency contact as required by the Standards. The father was listed as the second emergency contact. The Standards require that the name, address, and phone number of a person who can be contacted in an emergency if the parents can not be reached. 2. The father of child #3 was listed as not authorized to pick up the child. There was no legal paperwork in the child's record to support this. Plan of Correction: The front desk operator will have parents correct the records.
Standard #: 22VAC40-185-450-A Description: Based on observation, the center failed to ensure that the children were provided a top and bottom cover for their cots for rest. Evidence: 1. Five children in the toddler class did not have two covers for rest. All of the children were observed on their cots and resting with one cover. The staff stated that she was unaware of the requirement to have to linens (top and bottom) for the cots for rest time. Plan of Correction: The director will ensure that linens are provided today for rest period tomorrow.
Standard #: 22VAC40-185-540-D Description: Based on observation, the center failed to ensure that each first aid kit contained all required items. Evidence: 1. The first aid kit on the blue van was missing an ice pack. Plan of Correction: The driver will replace the ice pack today.
