Technical Assistance:
An exit meeting was completed by the licensing inspector with the center's director to review the findings of today's inspection.

The licensing inspector conducted an unannounced renewal inspection on Wednesday, November 15, 2017 at approximately 10:26 am to 4:14 pm to determine the center's compliance with licensing standards. During this inspection, the Director and Assistant Director were present and they accompanied and assisted the licensing inspector throughout this inspection. The licensing inspector was informed that the center is hosting a parent Thanksgiving luncheon today. The census for today's inspection consisted of 46 children. The staff to child ratios were deemed in compliance with licensing standards. The children were observed by the licensing inspector in the various classroom settings. The infants were observed by the inspector playing with age appropriate toys on the carpet, and engaging with staff. During this inspection, the children in the Toddler classroom were observed by the licensing inspector sitting at the designated table area with various books and interacting with peers and staff. The Early preschool classroom children were observed by the licensing inspector outside at the playground area and transitioning indoors into the classroom setting. The pre-kindergarten classroom children were observed by the licensing inspector watching Peppa Pig on the tablet and interacting. During this inspection, the staff were observed providing age appropriate supervision and providing assistance when needed. The licensing inspector interacted with both the staff and the children when appropriate. The children's lunch today consisted of turkey, mash potatoes, macaroni and cheese, green beans, pineapples, and milk. The center's classrooms were equipped with age appropriate toys and materials to meet the ages and stages of development in each setting. The bathrooms, kitchen, and playground areas were inspected today. During this inspection, the required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, emergency supplies, fire and shelter-in-place drills, liability insurance policy, and emergency preparedness plan. There are multiple staff with current MAT and Daily Health Observation training on site. Medication is not being administered at this time. There were five children?s records and five staff's records reviewed during this inspection.
The center's first aid kit and emergency supplies were inspected.

Standard #: 22VAC40-185-270-A
Description: Based on observations at the center, the center did not ensure all areas and equipment were maintained in a clean, safe and operable condition. Evidence: 1. During the inspection, there were multiple outside areas at the main entrance of the center observed with cracked wooden borders and protruding nails. 2. The school aged classroom areas were observed by the inspector unclean to include the floors covered in excessive debris and visible dirt throughout.

Plan of Correction: Placed a work order on 11/16/17 to replace boards in the main entrance and have an estimated time of completion by 12/16/17. We also had a meeting with all staff to review daily cleaning responsibility procedure and corrected all cleaning needs immediately.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, the center did not ensure a central registry finding for a staff was received within 30 days of employment. Evidence: In Staff#3's record (date of hire 03/20/17), the central registry finding document was dated 10/19/17 beyond the 30 days requirement. The inspector noticed there were three recorded dates (09/18/17, 09/21/17, 10/12/17) on the document in which it was received by the central registry department but required corrective action. The interview with staff revealed that the staff worked in classroom setting the week of September 11th, and the staff completed required training on 05/10/17 and participated in a staff meeting on 08/28/17. The central registry findings should have been obtained within 30 days of beginning employment.

Plan of Correction: We will review all staff files to ensure all required documentation is complete and updated on time in accordance with licensing regulations. We will also ensure all new hires are not scheduled until all required documentation has been submitted.
