Comments:
An unannounced monitoring inspection was conducted from 3:00 pm-4:15pm. There were no children in attendance due to parent/ teacher conferences. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on record review, documentation has not obtained that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Staff #1 was hired 8/24/17. There was no documentation of two or more references to include, documentation of dates of contact, name of persons contacted, the firms contacted, results and signature of person making the call.

Plan of Correction: I will design a form or use the State model form for collecting references. I will obtain 2 references for staff #1 and file them accordingly.
