A Subsidy Health and Safety inspection was conducted on November 7, 2017 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-009-2 Description: Based on record review and interviews, the vendor failed to inform the department's representative as soon as practicable, but not to exceed two business days, of a serious injury to a child while under the center?s supervision.
1. On September 20 , 2017, the vendor sent the Inspector an email detailing an accident that took place on the playground in which a child broke his collarbone.
2. The accident took place on September 15 and is past the two days requirement.
