Comments:
A Subsidy Health and Safety inspection was conducted on 11/27/17 to review supplemental health and safety requirements. Licensing Inspectors arrived at 11:40am and departed at 2:15pm. At the time of the tour, there were 18 children, ages two years old through four years old. Children were observed during rest period. The sample size was five children and five staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-B
Description: Based on a review of seven staff records and interviews, it was determined that the vendor failed to ensure that new staff complete the Virginia Preservice Training for Child Care Staff, to include all of the elements applicable to new staff, within 90 days of of employment. Evidence: 1. The record for staff #5 (date of hire 9/25/15), working during the inspection, did not contain documentation to demonstrate she completed the Virginia Preservice training. 2. Staff #1 (Program Director) stated that she could not provide any documentation to demonstrate that staff #5 had completed the Virginia Preservice training.

Plan of Correction: The facility responded: Staff #5 will complete the Virginia Preservice training. The Program Director will give deadlines and follow up with staff members to ensure that the Preservice training is completed.

Standard #: VENDSUB-000-012-E-1
Description: Based on a review of seven staff, it was determined that the vendor failed to ensure that all staff obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of the children within 90 days of the vendor agreement. Evidence: 1. The record for staff #6 (date of hire 9/8/17) did not contain a current certification for CPR. 2. Administrative staff verified that staff #6 did not have a current certification in CPR.

Plan of Correction: The facility responded: Staff #6 will complete a course to be certified in CPR. The Program Director will ensure that each new staff member is CPR certified within the first 30 days of employment.

Standard #: VENDSUB-000-012-E-2
Description: Based on a review of seven staff, it was determined that the vendor failed to ensure that all staff obtain current certification in first aid appropriate to the age of the children within 30 days of employment. Evidence: 1. The record for staff #6 (date of hire 9/8/17) did not contain a certification for first aid. 2. Administrative staff verified that staff #6 did not have a current certification in first aid within 30 days of employment.

Plan of Correction: The facility responded: Staff #6 will complete a course to become certified in first aid. The Program Director will ensure that each staff member is first aid certified within the first 30 days of employment.
