Comments:
A Subsidy Health and Safety inspection was conducted on 11/27/17 to review supplemental health and safety requirements. Licensing Inspector arrived at 10:40am and departed at 2:20pm. At the time of the tour, there were 45 children, ages three months old through four years old in care with 11 staff. Children were observed participating in various activities in the classroom, playing outside, eating lunch, during rest period. The sample size was five children and six staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a review of seven staff records and interviews, it was determined that the vendor failed to ensure that new staff complete the Virginia Preservice Training for Child Care Staff, to include all of the elements applicable to new staff, within 90 days of of employment. Evidence: 1. The record for staff #4 (date of hire 6/12/17) did not contain documentation to demonstrate she completed the Virginia Preservice training. 2. Staff #6 (Program Director) stated that she could not provide any documentation to demonstrate that staff #4 had completed the Virginia Preservice training.

Plan of Correction: The facility responded: Staff #4 will complete the Virginia Preservice training. The Program Director will give deadlines and follow up with staff members to ensure that the Virginia Preservice training is completed.
