Discussed lead teacher qualifications to consider for future hiring. Discussed anticipated request to modify license to include 2 year olds and also an increase in licensed capacity. Building Inspection Request form provided to complete and forward to the licensing office. Consulted on reviewing the first aid kit monthly as you complete the evacuation drills to ensure all items are maintained and operable.
An unannounced monitoring inspection was conducted this date. A total of 28 children with 5 staff and 2 volunteers were present. Today was picture day and also a Scholastic book fair. Two classrooms in use, both of which were observed. Children were observed completing table top activities or free play as they waited their turn for their pictures. Both classrooms were organized, clean and provided ample programming equipment for the children. A total of 8 children's records and 7 staff records were reviewed today. Standard #: 22VAC40-185-160-A Description: Based on staff record review, 1 out of 7 records reviewed did not have documentation of a TB screening. Evidence: Staff #1 was hired on 9/21/17. No documentation was found that a TB screening was submitted within 21 days of employment. Plan of Correction: We will obtain documentation of a TB screening for this staff.
Standard #: 22VAC40-185-240-C Description: Based on staff record review, 1 out of 7 staff did not receive the required 16 hours of annual training. Evidence: Staff # 4 was employed 9/1/15. From 9/16 to 9/17, Staff #4 received a total of 10 of the required 16 hours of annual training. Plan of Correction: Director plans on providing monthly training for all staff which would allow staff affordable and on site training.
Standard #: 22VAC40-191-60-B Description: Based on staff record review, 2 out of 7 staff records reviewed did not contain a complete Sworn Statement. Evidence: Employee #1 was hired on 9/21/17, the Sworn Statement maintained in the record was not current which includes a question about residency outside of Virginia. Employee #2 was hired on 8/20/17, the Sworn Statement maintained in the record was not current which includes a question about residency outside of Virginia. Plan of Correction: We will have all staff complete the new Sworn Statement form.
