Comments:
An unannounced monitoring inspection was conducted on November 28, 2017. There were 40 children present with five staff members. Additional staff were on site for administrative, volunteer and support purposes. The children were observed napping and the children who were awake were observed engaging quietly with staff . The posted menu indicated that today's lunch consisted of turkey sandwiches with lettuce and tomato, vegetable soup, baked apples and milk. For afternoon snack the children would be served carrots with ranch dip and milk. Five children's records and three medications were reviewed during the inspection.

Violations:
Standard #: 22VAC40-185-440-E
Description: Based on observation and inspection of the facility, the staff members did not ensure that there shall be at least 12 inches of space between occupied cots. Evidence: During the inspection conducted on November 28, 2017, in Classroom #2, the licensing inspector observed four occupied cots with less than 12 inches of space between them.

Plan of Correction: We will review the spacing between the cots.

Standard #: 22VAC40-185-550-M
Description: Based on review in two of five injury records reviewed, the licensee did not ensure that each written record of children's serious and minor injuries were documented with all required information. Evidence: During the inspection conducted on November 28, 2017, the following information was not documented on each written record of children's injuries: Injury Report #1 did not include documentation of the time the parent was notified of the injury Injury Report #5 did not include the date of the child's injury

Plan of Correction: We will remind the staff to put in dates and times.
