A Subsidy Health and Safety monitoring inspection was conducted on 11/20/2017 to review supplemental health and safety requirements. This inspection began at 12:25pm and ended at 4:00pm. There were 15 staff with 102 children in 11 classrooms at the time of the visit. At the start of the inspection, children were observed napping, waking from nap, playing in small groups on tablets, arriving from school and playing outside. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Description: Based on record review, a written allergy care plan was not obtained for all children with a diagnosed food allergy. Evidence: Child #4 is diagnosed with a food allergy and the allergy care plan is not completed. The plan that is in the child's file does not contain the steps to be taken in the event of a suspected allergic reaction. The plan is not signed by the child's physician. Child #5 is diagnosed with a food allergy and the allergy care plan is not complete. The plan in the child's file does not contain the steps to be taken in the event of a suspected allergic reaction.

Plan of Correction: A request for additional information regarding a plan of action for a suspected allergic reaction has been submitted to physician.
