An unannounced Subsidy Health and Safety Inspection was conducted on November 27, 2017 from approximately 4:30 p.m. until 6:00 p.m. Upon entrance there were 21 children and 3 staff present with children. The record review sample consisted of 5 children's records and 3 staff records. Children were observed playing board games, Legos and homework.
Standard #: VENDSUB-000-006-B-11 Description: Based on record review and interviews, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on November 27, 2017, 5 children's records were reviewed and 1 did not include a written allergy action plan for child #2 who has a diagnosed food allergy (fish, shellfish and eggs) and there was not an action plan available for review. Plan of Correction: The center director will have the parents obtain a plan.
Standard #: VENDSUB-000-012-E Description: Based on record review and interviews, the vendor did not ensure all staff providing care to children had obtained within 30 days of the date of employment or 90 days from subsidy vendor approval CPR and First aid certification. Evidence: During the inspection on November 27, 3 staff records were reviewed and the record for staff #1 did not include CPR and first aid certification. Center staff confirmed staff #1 had been employed since October 13, 2017. Plan of Correction: The center director will have staff take CPR and first aid.
Standard #: VENDSUB-000-031-B-1 Description: Based on a review of documentation and interviews, the vendor did not ensure the evacuation plan procedures included accommodations or special requirements for children with special needs to ensure their safety during evacuation or relocation. Evidence: During the inspection on November 27, 2017, the emergency preparedness plan did not include accommodations for the special needs children in the program. Plan of Correction: The center director will write the plan within a week.
